ATHLETIC PHYSICAL FORMS

All incoming 7th grade students are required to have a school physical completed AFTER MAY 1st prior to entering 7th grade. This will count for PE and if your student chooses to participate in a school sport.

If your student chooses to participate in school sports they will have to have another school physical again in 9th grade and their 11th grade year.
